Job Description

The Guernsey County Senior Citizens Center is hiring a Wellness Coordinator.


Primary job duties and responsibilities include but are not limited to: 

  • Instruct Wellness Classes; Matter of Balance; Chronic Disease Self Management; Diabetic Disease Self Management; Tai-Chi; Geri-Fit; all other wellness programs and classes.
  • Oversee and manage all aspects of the Senior Supplemental Food Commodities Program (SSFCP) including, but not limited to, administering, reviewing, and approving applications & renewal applications {Annually}.
  • Responsible for ensuring adequate volunteer support during distribution dates.
  • Responsible for ensuring all Mid-Ohio Food Bank documents, reports, files, records, and information are properly submitted, filed, and completed.
  • OBTLSS Referral System.
  • Assist with managing and maintaining participant files for the following programs:

Ø Information and Referral  - Code 18-15

Ø Telephone Reassurance    - Code 16-29

Ø Legal Aide                       - Code 13-06

Ø Health Assessments         - Code 09-02

Ø Senior Citizens Center Membership

Ø HEAP Applications

  • Assist with Newsletter preparation & Sort Mail
  • Assist with Answering telephone calls (Back up to Main Receptionist-breaks, lunch, absences)
  • Serve as Wellness Coordinator working in conjunction and in support of the Activities Coordinator
  • Assist Activities Coordinator as needed, assigned, delegated, or as instructed
  • Assist with developing in-house Wellness related programs and activities {i.e. exercise, tai chi, walking, diet plans, health fairs, information sessions & series, “Wellness Corner”, recipes, social groups, etc…}
  • Assist with coordinating and scheduling activities & events.
  • Assist with Golden Buckeye Card Applications
  • Assist with preparations, decorating, and advance details for activities and events; and/or as assigned and delegated by the Activities Coordinator
  • Maintain proper accounting and donation record keeping practices
  • Maintain membership, mailing address, and other client databases (computer) 
  •  Annually update all participants’ files of the aforementioned services as designated or required.
  • Assist with assembling and distributing monthly newsletter.
  • Assist with mailing postcards monthly to remind membership participants to renew their annual memberships.
  • Answer Senior Center Phone Lines and provide assistance to the caller. If call pertains to one of our programs transfers call to proper supervisor.
  • Completes and verifies information for Buckeye Card Application.
  • Completes and verifies information for HEAP, Food Commodities, Legal Assistance, Telephone Reassurance, and Senior Citizens Center membership.
  • Assist Activities Coordinator in rental of Senior Center for wedding receptions and other in-house events.
  • Will complete all documentation as required by ODA, AAA9 and the Administrative Assistant or Executive Director.
  • Will perform any other duties as required by the Activities Coordinator, Administrative Assistant or Executive Director.

Will report directly to the Activities Coordinator.

 

Education / Requirements:

High School diploma or equivalency - Health/Wellness Related Certifications - Strong clerical skills - Sensitive to needs of Senior Citizens

 

Additional Job Knowledge:

  • Must be organized, efficient, and very courteous and respectful to clients and customers at all times.
  • Shall receive ½ hour paid lunch break during a five-hour workday.
  • Shall receive paid holidays after successful completion of 90 day probationary period.


Mandatory Annual Trainings:

  • CPR & First Aid Training
  • Drug Free Workplace Training
  • Pre-Post Employment Drug Testing
  • BCII Criminal Background Check
  • HIPAA Training
  • Additional trainings as needed and/or required.
